Guardian offers a choice of two High Deductible Health Plans (HDHPs). Our plans include a large network of doctors and health care providers, as well as discounts if you decide to use an out-of-network provider. You’ll receive routine preventive care at no cost, while other services like sick visits at a doctor’s office, emergency care, and hospital stays are covered at the plan deductible and coinsurance levels.
When enrolling in Guardian’s HDHP, having money available for both planned and unplanned health care expenses can be important. You’ll have a choice to participate in a Health Savings Account (HSA) or Health Reimbursement Account (HRA) to help offset your out-of-pocket costs. The HSA offers tax free contributions and withdrawals for qualified health care expenses, easy access to savings, future growth potential, and the ability to roll over funds year to year. Depending on the medical plan you enroll in, Guardian may contribute to your HSA. As an alternative, the HRA is fully funded by Guardian, allows reimbursement of qualified medical and prescription drug expenses, and allows a limited roll over of funds to the next year.
When you enroll in our medical plans, you’ll automatically be enrolled in prescription drug coverage. The plan covers generic and brand name medications filled at both retail and mail service pharmacies.
Our plan offers a network of approved dental care providers and emphasizes routine preventive care offered at no cost, with coverage for other non-preventive services. The plan also features a Maximum Rollover Account (MRA), which helps you save your annual maximum dollars for a time when you may need them the most.
Guardian offers two vision options for you to choose from. All options offer in- and out-of-network coverage for routine eye exams, and glasses or contact lenses. All vendors offer coverage at a private practice or retail chain provider.
Note: Avēsis employees have access to a separate vision plan.
When you start with Guardian you’ll automatically be covered with disability income insurance, that provides a portion of your income for both short-term and long-term periods, if you can’t work due to illness or injury. We also offer additional long-term disability coverage that you can purchase to help protect more of your income.
On your first day at Guardian, you’ll be enrolled in life insurance coverage at no cost to you. You can purchase additional term life insurance coverage for yourself and your dependents, or you can take advantage of our employee-only discount on whole life insurance.
Guardian provides a 401(k) plan that allows you to save for retirement. You may elect to save on a pre-tax, post-tax, or Roth basis. Guardian adds to your savings by making Company contributions to your 401(k) account.
