Notice to Employers

NY Regulation 200 requires insurers to request and, if provided, retain, at a minimum, the name, address, date of birth, social security number, and telephone number of every insured (including dependents) with life insurance coverage and beneficiary. The enrollment and beneficiary information provided to Guardian reflects your choice of plan administration. To process claims for life insurance benefits for your covered employees and their covered dependents, Guardian requires that you complete the Employer Statement portion of the claim form, verify the claimant’s eligibility for coverage, and provide the claimant’s current beneficiary designation upon initiation of the life insurance claim.
