What are the requirements to offer this?

To offer the 1:1 enrollment experience, an employer must:

Offer at least one Guardian voluntary benefit.

Complete setup at least three weeks prior to the enrollment period (after all required information is received).

Utilize an online enrollment platform or Guardian Enrollment powered by Flock.
