How to file a life insurance claim
Follow these steps to submit a life or accidental death & dismemberment claim in the unfortunate event that an employee or one of their covered dependents passes away.

Go to Guardian Forms and Claims and select “Find a form.” In the Form Number boxes, select “GG” and type “42,” then click “Search.”
Access the Group Life Claim Form (GG-42) to complete the form online or print it out. You will need to complete sections 1-3 and the beneficiary needs to complete section 4.
Submit the completed form along with the required claim documents via one of these methods:

Planholder Life and AD&D claims checklist
1 All whole life insurance policy guarantees are subject to the timely payment of all required premiums and the claims paying ability of the issuing insurance company. Policy loans and withdrawals affect the guarantees by reducing the policy’s death benefit and cash values.
2 Some whole life polices do not have cash values in the first two years of the policy and don’t pay a dividend until the policy’s third year. Talk to your financial representative and refer to your individual whole life policy illustration for more information.
3 The Disability Income and Waiver of Policy Premium Benefit Rider (form ICC21 DIR, DIR (12-2021), or state equivalent) is underwritten and issued by The Guardian Life Insurance Company of America (Guardian®), New York, NY.
