Network dentists are at the heart of our dental business and helping them ensure safe access to quality oral care continues to be our key focus.

Considering the extraordinary circumstances of the COVID-19 Pandemic, Guardian is offering financial relief for certain Personal Protective Equipment (PPE) expenses incurred by network providers during the months of August through October. 

  • PPO providers and DHMO specialists will receive $8 per unique patient visiting a dentist on a specific date from August 1 – October 31, 2020.
  • DHMO primary care dentists will receive $2 for the average number of Guardian members and dependents assigned to their office between August 1 – October 31, 2020.
  • The minimum combined payment to any network PPO and/or DHMO provider will be $25.

    How to qualify for PPE expense contribution

    • Participate in Guardian’s PPO or DHMO networks
    • Register or log in to Guardian Anytime to sign up
    • Be signed up for Electronic Funds Transfer (EFT) for this payment and provide your bank routing and account numbers when you enroll

    How do I enroll?

    • To enroll, log in or register at Guardian Anytime. If you are not yet registered for Guardian Anytime, you must register prior to signing up. Dental Practices can enroll by providing Tax Identification Numbers (TINS). All providers under that TIN will be enrolled.
    • Once logged in, click the link on the right-hand side of the landing page to sign up for PPE contribution funds.
    • Make sure to provide your banking information at enrollment to receive this specific payment. 

    Can I enroll if I am not in Guardian’s network?

    • You must join our network by October 31, 2020 to participate. To join, complete the form here or contact us at 1-866-229-1970.
    • You must remain in network for the duration of the program with no active termination letter on file.

    Is there a deadline to enroll for PPE contribution?

    You must enroll by October 1, 2020.

    What information will I need to provide when I enroll?

    Have this information ready when you apply:

    • Tax ID Number (TIN)
    • National Provider Identification (NPI)
    • Bank routing number
    • Account number
    • Practice email address 

    How will PPE contribution be determined?

    • PPO providers and specialists in our DHMO network will receive $8 per unique patient visiting a unique dentist on a specific date of service. Payment will be based on submitted claims from August 1 – October 31, 2020. Claims must be submitted and processed by November 30, 2020. Claims submitted after this date will not be included and no exceptions will be made.
    • DHMO primary care dentists in our network will receive $2 for the average number of Guardian members and dependents assigned to their office between August 1 – October 31, 2020.
    • The minimum combined payment to any network PPO and/or DHMO provider will be $25.

    Does the PPE contribution apply to all my patients with Guardian dental insurance?

    • Our customers with Administrative Service Only (ASO) plans are invited to opt in to participate in this program.
    • If they elect not to participate, the PPE contribution will not apply for their employee-member dates of service.

    When will I receive the PPE contribution?

    You will receive a one-time payment via Electronic Fund Transfer (EFT) in December 2020.

    How will I receive the funds?

    • You will need to be signed up for Electronic Funds Transfer (EFT) for this specific payment and your bank routing number and account number for payment.  
    • If you would like to use EFT for your benefit payments with Guardian moving forward, please visit www.changehealthcare.com to sign up.
